CAUSE AND EFFECT ESSAY Cause and effect essays are usually taught in high school and introductory college writing courses. This type of essay does just as the title suggests; it explains the cause(s) of a problem and details the resulting effect(s). These few steps can help you write this type of essay. Suggestions: Step 1: Present an event, phenomenon or trend. This will be the cause and the starting point of your essay. You should explain the background of the cause in significant detail so your reader understands the basis of your paper. Step 2: Explain the effects of your event, phenomenon or trend.You should continually refer back to the cause to make links and connections between the cause and effect. Links and connections will help your audience effectively process the cause and effect. Step 3: Limit your major points. While there may be multiple causes or multiple effects in any given relationship, limit your points depending on the length of your essay. Too many points will only confuse your reader. SPECIFIC PURPOSE:At the end of my speech, my audience should understand three important points of the history of Halloween. THESIS STATEMENT:The three most important points of Halloween can be summed up by looking at its origins, how it came to include jack-o-lanterns and bobbing  for apples, and how it is celebrated today in the 21st century with  trick-or-treating and haunted houses. When the leaves start turning different colors and falling off the trees, when the temperature starts falling and the sun starts setting a little earlier each day, it makes me think of Halloween. Most people think all Halloween is about is dressing up and going trick or treating. It didn’t start out as a going door-to-door and getting candy event every October 31st. In fact, Halloween originated as a Celtic festival more than 2000 years ago. When I was a kid, Halloween was my favorite time of the year. It wasn’t even because of all the candy and the dressing up, but for the overall atmosphere of it. I have always loved all things scary and fall has always been my favorite season so naturally every year I look forward to the end of October every year. The three most important points of Halloween can be summed up by looking at its origins, how it came to include jack-o-lanterns and bobbing for apples, and how it is celebrated today in the 21st century with trick-or-treating and haunted houses. I.Beginning somewhere around 800 BC, The Celts celebrated â€Å"Samhain†(pronounced â€Å"sow-in†), according to the book entitled â€Å"The Celts† by Nora Chadwick. A. Samhain is a festival to recognize the end of summer. 1. The Celtic celebrated Samhain near the end of our month of  October, which was the end of the year for them. 2. It often involved a big feast because it was the end of harvest also. B. The Celts believed that the veil between this world and the next was thinnest at this time of year. 1. Friends and relatives who had died would often return, with their souls inhabiting an animal – often a black cat. 2. Black Cats have remained a symbol of Halloween even today. C. In celebration of the recently completed harvest, Celts would give offerings of food to the Gods. 1. They often went from door to door to collect food to donate to their deities. 2. Also, young Celts would ask the townspeople for kindling and wood, and take it to top of  the hill for the Samhain bonfire. 3. These are the two possible origins of the modern â€Å"trick or treating† ritual. 1. Sacred bonfires were lit on the tops of hills in honor of the Gods. 2. The townspeople would take an ember from the bonfire to their home and re-light the  fire in their family hearth. 3. The ember would usually be carried in a holder, usually a turnip or gourd. E. The Celts felt nervous about walking home in the dark on account of the evil spirits. 1. They dressed up in costumes and carved scary faces in their ember holders. 2. They hoped that the spirits would be frightened and not bother them. 3. This is why we carve pumpkins and children dress up for Halloween. II. For the next eight centuries, the activities going on at the end of October began to change. A. According to website â€Å"The Origins and History of Halloween† 1. Jack was mischievous Irishman that had tricked the Devil into climbing an apple tree. 2. He then cut a cross symbol in the tree trunk, thus trapping the Devil in the branches. 3. At his death, he was unable to again access to Heaven because of his meanness and the  Devil would not allow him into Hell because of the apple tree incident. 4. He was forced to walk the earth endlessly but the devil took pity on him and gave him a piece of coal to light his path. 5. Jack put the coal inside a hollowed-out turnip that he had been eating. 6. This is the reason we light â€Å"jack-o-lantern† pumpkins on Halloween night. B. Apples were considered have long been associated with female deities, and with immortality,  resurrection, and knowledge. 1. One reason is that if an apple is cut through its equator, it will reveal a five-pointed star   outlined at the center of each hemisphere. 2. This was a pentagram — a Goddess symbol among the Gypsies, Celts, Egyptians, 3. Unmarried people would attempt to take a bite out of an apple bobbing in a pail of water, or  suspended on a string. 4. The first person to do so was believed to be the next to marry. 5. This is where the ritual of â€Å"bobbing for apples† originated. C. All Saints’ Day was a holiday to recognize the saints who were without their own day, and to  celebrate saints that the Church had failed to recognize. 1. It originally was held on May 13, but was moved to November 1, possibly to distract  Christians from celebrating Samhain. 2. Halloween was originally called All Hallows’ Eve which means the evening before All  Saints’ Day. 3. â€Å"Hallow† is an Old English word for â€Å"saint† III.Starting in the 20th century through present day, Halloween has become a major folk holiday in A. Trick–or-Treater’s go from door to door and collect candies, apples and other treats. 1. Halloween is the holiday when the most candy is sold. 2. It is second only to Christmas in total sales of any holiday. B. Scares and spookiness are a big part of Halloween. 1. Local community organizations put on fundraising events like haunted hayrides, scary  walks through the woods, etc. to raise money for their programs. 2. Privately-owned organizations take old houses or abandoned buildings, throw in a few  scary monsters here and there to create a haunted house. C. Hell or Judgment houses are a relatively new concept created by conservative Christian sects. 1. The earliest hell house appears to have been created by Rev. Jerry Falwell in the late  1970’s, according to the â€Å"Halloween Spooking, Christian Style,† American Atheists  news release, 2001-OCT-27. 2. A Hell House consists of a group of horrific presentations within a type of haunted  house where a customer walks through a sequence of scenes designed to create terror  and revulsion. 3. The intent is to convert the unsaved public  to Christianity and to promote certain  conservative Christian beliefs like the wrongs of abortion, homosexuality, and sex  before marriage. 4. The last scene is different, typically a portrayal of heaven where visitors are then asked  to accept salvation by repenting of their sins and accepting Jesus as Lord and Savior. Having seen how Halloween in modern times has evolved from its humble beginnings as an end of the growing season celebration, it is interesting to see how much festivities centered around the 31st of October have changed over the last 2000 years. It has gone from an end-of-year festival around 800 BC to a time when kids go door-to-door around their neighborhoods dressed up like their favorite cartoon characters. Halloween is just a fun day for everybody. It can bring out the kid in all of us.

Essentially the patient has been seen to receive some form of treatment that he or she believes will work. This is a psychological response that can reduce stress and panic thereby leaving the bodys true immune system with an improved chance of working. Placebos are not a cure but merely help to place the patient in a more relaxed and receptive state for treatment. In this regard all medicines essentially provide what is known as the placebo effect. In general terms the placebo has a positive impact if managed by physicians in a proper and responsible way. This is because stress and anxiety are known to affect the body in an adverse manner and possibly make the symptoms worse. Researchers have found that large dummy placebo pills that are coloured seem to stimulate a more positive response from the patients. The important point to emphasise is that the placebo effect is practiced in all medicines and whilst some consider it to be a bogus form of treatment, nevertheless, it accomplishes more good than harm. Counter arguments state that the feel good generation is purposely misleading and could create more serious conditions. The argument is somewhat flawed as very rarely is it ever used as a standalone form of treatment but more a precursor to getting a patient stabilized in a relaxed and more receptive state in order to receive proper medical attention. The medical journal The Lancet provided on an article that stated a new analysis of 110 placebo-controlled randomized trials of homeopathy, indicated there is no evidence that homeopathic tablets perform any better than placebos (Goldacre 2006)  [1]  . Modern medicine has moved towards better patient relationships keeping the patient informed and moved away from the old paternalistic placebo doctrine. Most doctors are uncomfortable with dishonesty and perhaps this is one of the key distinctions against that of the homeopathy practitioners. Adam Harrington stated in Hoboken that the placebo is much more ado about nothing the answer is NO because of the powerful therapeutic effects and YES because of the faddish exaggerations about placebo power. (Harrington 1999). The term placebo is Latin and originated from the Catholic vespers for the dead and translated means I shall please. (Evans 2004) One of the most convincing positive arguments for placebos resulted from the work of Dr. Henry Beecher  [4]  who was the American anaesthetist who treated US soldiers in a field hospital in World War 2. In one instance he was treating a soldier with bad injuries and was considering morphine as a pain killer but he was concerned about cardio vascular shock. Instead he put the soldier on saline solution and the result was the patient settled down, he felt less pain and the reaction was similar to morphine. It seemed that salt water could be as effective as morphing considered one of the most powerful painkillers at the disposed of the medical practitioners. Beecher later repeated the trick with the same results and after the war returned to Harvard University to continue his research. Similar studies were being carried out at Cornell University. The subsequent studies carried out by Beecher, gold and lasagne change the way the medical profession viewed placebos and by the mid-1950s doctors were thinking of this as their longer a fraudulent practice. The belief concept may well i nfluence the chemical releases within the body that are controlled by functions of the brain. Tests with depressive patients were conducted by doctors using brain scans and where there was an expectation by the patient that the condition would improve there was a marked increase in biological changes to the brain. Hence the biological stimulation of the brain is strongly linked to the potency of placebos. People who have been suffering postoperative pain have reported considerable improvement in pain reduction after a simple saline injection. These analgesic effects suggests suggest some form of reception by the brain; this has been referred to as activation of the endogenous pain-relief system of the brain (Connors W 2007). The brain itself manufactures morphine like substances called endorphins the brain can trigger or send these to those parts of the body experiencing pain. Hence placebos may be able to stimulate this part of the brain in order to enhance that stimulation process . Placebo affects are not limited purely too chemical or drug treatments, they have also been included in both surgery and physiological therapies. A summary of the many literature works on placebos leads towards a number of preliminary conclusions:- [1] A significant number of patient show a therapeutic response from the administration of a placebo [2] There seems to be no uniform practice that sets itself out as a prescribed means of treatment in this area [ 3] It appears that any object with therapeutic intent can invoke similar circumstances [4] The mechanism of the effects remains relatively unknown and the conditions in which it operates are unclear. (Wickramsehera 1988) CONCLUSIONS The advantage that supports the use of placebos are essentially threefold: [1] the mind over matter concept whereby the placebo acts as an assist to the bodys natural defence mechanism [2] the provision of a psychological boost or stimulus to the brain which may prove to be an important contribution to the overall recovery of the patient [3] A support to the central concept of belief which is known to be a major contributing factor towards recovery. The main disadvantages might be summarised as [1] In the area of chronic pain the patient may show an immediate spontaneous improvement and as such give cause to a false diagnosis of the problem [2] The element of deception may provide the patient with false hope or expectations and give rise to medical ethical considerations [3] The patient may provide the Doctor with erroneous information believing that the placebo has resolved the problem. This loses valuable time in the determination of the correct prognosis and could prove serious in certain cases. There is also the strong argument that placebos can become habit forming. One example being the use of valium for strong headaches. These may essentially provide temporary relief but they are certainly not an ongoing solution. It is the unsettling notion that people are getting medical treatment from unqualified practitioners. The concept of non-medically trained people prescribing any form of placebo implies that the patient is ill and as such this can be a powerful message to the patient. The action of medication is not just controlled by the drug companies as the pharmaceutical companies have to undergo extensive clinical trials before they can release a product to the market place. A lot of the controversy with the applied use of placebos is around the use of these in clinical trials. Many consider that the use of these is unethical but it raises the question as to whether the trials could be successful without them Arguments against the use of placebo groups in clinical trials have been based on opinion rather than evidence. Ethical issues have been raised, but these are contentious. (Martin Tramer 1998). Leaders have to be focus all the time. They have to keep on remind themselves and the team members of the goals and mission. As leader of the group, it is important that you schedule time to meet with your team to establish and check-in about the goals you hope to achieve. Leaders also have to be organized. Leaders are responsible in a lot of things and might be very busy sometimes. A leader can set the tone for the team. A leader who is organized helps motivate team members to be organized as well. As a leader, youre responsible for a lot and youre probably going to be very busy at times. However, you still need to find time to talk with your team. A good way to do this is to set frequent group meetings, so that no question or concern goes too long without attention. Leaders also have to be decisive. Although an important part of being a leader involves listening to the people surrounding, they have to remember that they are not always going to be able to reach a compromise. When this happens, dont be afraid to make the final decision, even if some team members disagree with the plans. Confident is the most important characteristics of a leader, leaders have to believe themselves and the success of the team members. Show others that you are dedicated, intelligent, and proud of what you are doing. A manager basically directs resources to complete predetermined goals or projects. For example, a manager may engage in hiring, training, and scheduling employees in order to accomplish work in the most efficient and cost effective manner possible. A manager is considered a failure if they are not able to complete the project or goals with efficiency. . On the other hand, a leader within a company develops individuals in order to complete predetermined goals and projects. A leader develops relationships with their employees by building communication, exampling images of success, and by showing loyalty. As an example of a manager, A company CEO directs Wong, one of the companies up and coming managers, to hire enough new employees to provide the company with a customer service department. Wong undertakes his project with enthusiasm. He hires only those employees who can work the assigned hours, will accept the modest pay, and have experience working in customer service. He trains his new employees to perform the job to his expectations and assigns the employees to their new positions. Wong measures his success in terms of efficiency, calls handled per hour, and cost effectiveness, for example did he meet his budget?. However, Wong did not anticipate that of the employees he hired, only a handful would remain working six months later. Moreover, as an example of a leader, Ahmed obtains the same assignment as Wong. Ahmed hires employees that he believes he can develop a working relationship with, versus just those employees who will worked the assigned hours and take the modest pay. Ahmeds goal is to hire a diverse group of employees, some of who do not have any customer service experience, who he feels he can develop a personal connection. A large part of Ahmeds training involves team building, telling successful stories, and listening to each employees own desires for what constitutes a fulfilling job. Ahmed still assigns his employees their job duties and schedules at the end of training, and he also measures success in terms of efficient and cost effectiveness, but he also measures success in terms of low employee turnover, employee morale, and employee development. Ahmed feels proud when one of his employees obtains an advance level position a year or two after being hired. The skills to be a leader or a manager are not exclusive in nature. A leader who only displays leadership skills will be ineffective when it comes to checking time cards, completing employee reviews, and scheduling employee vacation time; things that employers require their managers to do on timely bases. Similarly, a manager who spends all his/her time completing paperwork and reading reports; only creates more problems for him or her because they lack a developing relationship with their employees. Many companies, one person may play the role of both leader and manager. In others, these roles are carried out by different people. And in many cases, the roles overlap and leaders need to manage and managers need to lead. However, it is critical that both roles are performed effectively for a company to succeed, especially a company with employees. If that is the worst thing I have to be worried with when I’m older, then I’m not too scared about getting older or being single. On many T.V. shows the main focus of an episode has to do with a guy looking for a girlfriend, or girl looking for a boyfriend. That doesn’t happen in real life. I mean sure people look for dates of to get hooked up, but it doesn’t occupy their whole mindset. It seems that on T.V. single people have no responsibilities. They always focus on insignificant materialistic things. Clothing, jewelry, and money are the main topic of discussion or concern. Single people seem quite independent on shows. They appear free to run around freely, unburdened by commitment. On â€Å"Friends† they seem to have plenty of time to sit around and talk about things all day long. And of course there is â€Å"Seinfield†, the ultimate show about absolutely nothing. None of them are married, but they are portrayed as living perfectly happy with the smallest of problems. Life to some people is a long learning lesson and process. Everyday is a learning experience and a novel opportunity to expand our knowledge and information bank, we stock piles our minds with both vital and non crucial information, hoping to utilize it as time goes by. This is a philosophy that reluctantly I have come to ascribe to. Looking back at my life, no matter how short it has been, reveals a rather extensive period of learning. I am a 23 years old girl, born in Korea and just immigrated to the U.S.A two years ago in a bid to better my life and pursue my dream career. Growing back in the suburbs of Seoul from my childhood days, through to my adulthood, is itself a thrilling experience. Although lacking in some luxuries and carefree attitude available in many American cities, I cherish the sweet memories of the good times I have gone through in Korea. I was born and brought up by my two staunch parents, who partook their parenting duties with the strictness and seriousness it deserves. To them, bringing out the best in me, as their only child, was their sole calling. No resource would be spared in ensuring that their only daughter got the best that life could offer and emerge triumphantly. Whether or not I have come out successfully is another story altogether. Many will agree with me that their childhood days though full of bliss and joyous moments, was not always a bed of roses, especially if you had an uncompromising mother or a nanny watching behind your back in every move you made. I came to agree that they meant well but our naivety and simple minds could not fathom the reason why we could not be allowed to do what we wanted. I take a look at my life and appreciate that I have come from far. Importantly though, is that the best that I am today, I owe it to my adorable parents. It is said that, cleanliness is second to godliness-that I have come to see is true. I have received compliments and encouraging words regarding how organized and neat I am, from my teachers, friends and even strangers. Many however do not know that this has not always been the case, as my parents can attest. My young urchin life is a complete opposite of the tidiness and smartness I exhibit today. I grew up in a neighborhood inhabited by diverse people and hence diverse characters. My immediate neighbors were two boys whose sweet memories I still cherish, however, the care-free attitude they had instilled in me than is a trait I would not have wanted to acquire. It is a known fact that kids can be extremely naughty especially in the wrong environment. My mother had always been trying to emphasize the importance of neatness especially in girls, trying to buy me bright colored dresses to ensure that I looked my best. I could not hear anything of it and was always in crumpled clothing and uncombed hair. By the age of 12, my mother had given up. Not even my father’s smart and stern look could change me. I used to be extremely untidy both at home and in school. My homework was not being delivered in time and when delivered it was just a bunch of illegible scribbling. My parents had a perfectionist attitude towards life, but that too could not change me. Every road has a turning point and every night a daybreak. My annoying habits too had to come to an end. This came at the night of my 15th birthday. Since the tender age of three years, it had been a tradition in our house to hold my birthday party where I would invite my friends for a day full of celebrations. I had wished to see a different thing happen this 15th birthday. At my age, it was considered trendy to hold a birthday party at night and let your friends enjoy an orgy till day break. I conferred with my parents and they had no problem with that. However, it was on a condition that our house nanny would not play a role in tidying up the house before or after the party. I had to somehow find a way of cleaning the place. How I tidied up my room is a story for another day but I did it altogether, and the results were quite impressive. My friends had always thought of me as a disorganized person with no cleanliness ambitions; it amazed them that I could be that tidy. My parents were quite impressed and I could see a sense of renewed affection in them. I was hoping to maintain this and made it may birthday resolution; since then I have never looked back. I can tell when my parents are relieved and happy. After my tidying up I could tell they had renewed their hope in me. Their next concern was to be in academics where I had been recording a dismal performance over a long time, a performance that had emanated from the fact that I disliked studies. I found the idea of devoting my precious time in abstracts a laborious task. This had to change somehow. My dad is not only smart in terms of dressing, but it also very knowledgeable. This was a trait he had all along wished to instill in me. He started by buying me short story books to try and arouse more interest in me. This is what eventually would see me develop an unquenchable interest in reading and digging for knowledge in topics ranging from almost every aspect of life. I have come to like reading both fictional and scholarly materials especially those authored by famous people. It is in reading widely that one gets to know more about the world. I have come to appreciate the role my parents have played in my life. They have taught me the essence of grooming well and proper self-organization as a key ingredient to succeed in life. My academic life has improved greatly as a result of the important tips accorded to me by father. The novel, Bless Me ultimo by Antonio Mare, tells a story of a young boy named Antonio who lives in New Mexico with his family. These conflicting impressions make Antonio question his future. One day he hears the story of the golden carp from his friend Samuel. The story is about how a group of people angered the gods and they turned all the people into carp.But one god loved the people so much that he asked to be turned into a carp as well. Because he was a god they made him very big and made him of gold. After the story, Antonio states â€Å"The Golden Carp†¦ A new God (peg 129). † This shows a moment of realization for Antonio because he knows that not everybody is catholic and God has a different meaning to everyone. Quickly his ideas about God and religion start to shift. As the novel continues, numerous moral questions begin to arise as he sees more death and hatred. It is much more than he has every experienced before.His mom wishes to hide him from the hateful things that occur outside his door, but she realizes she can only do so much to protect him. He will have to figure out the ways of life for himself. He gains wisdom from the people around him and he learns that to gain happiness because he must be at peace with his identity. References Introduction This is a report on Gary a 52 year old male service user who lives within the county of Conway. Due to a serious car accident some time ago he has lost the use of both his legs and he does not work and relies on disability benefits for his income, Gary lives alone and has some care provided by his daughter and the rest by a support worker.This report has been commissioned by Gary with the help of his support worker as he is interested in taking up some retraining so he can move towards getting back into the job market and becoming more independent. His daughter and son in law are apposed to the idea as they think Gary is getting on in years and she would not be able to attend the address to get Gary ready for work before 9. 30 am as she needs to get her children to school.His daughter also has reservations about the Gary's ex-work colleague as he has a reputation of being quite demanding of his employees and pays only a basic wage. In this report we are going to look into all the options that are open to Gary as in regard to him getting extra care so that he is able to push towards his goal of retraining for work. The key issues that we need to address as regard to Gary are can we get him the extra care that would make him more independent and give him the mobility to let him attend further education and training to put him back in the work place.This report is being compiled for Social Workers that are based within the Adult Social Services within Conwy County Council. Context With Gary's situation a number of significant events have happened which have led to his current situation, he lost the use of his legs sometime ago and lives alone after getting divorced some 15 years ago. Gary is suffering from depression and is taking medication for this, his daughter and son-in -law are relied on for most of his care and a support worker attends each day to give him a bath.Since the onset of his depression Gary has withdrawn from doing the activities that he used to enjoy and he is very much cutting himself off from the world. When looking at Gary's situation I am applying the Social Model of disability which was created by disabled people themselves.This was primarily created as a result of the response that society has given to disabled peopl e and also a reflection of the experience that disabled people have come across in the health and welfare system which as given them a feeling of being socially isolated and oppressed. The second perspective that I am going to apply to this report is empowerment, this gives the service user greater control of the services that they receive, being involved in the planning of services and developing good practice. (Barnes and Bowl, 2001) DiscussionIn the case study that this report is based upon Gary has been in a serious car accident that has deprived him of the use of his legs and this now means that he can only get around with the use of a wheelchair. This situation has had a knock on effect of him have a low sense of self-worth and a lack of pride in his situation, he suffers from depression and is taking medication for this. Radar (the disability network) and Depression Alliance commissioned a report (http://www. depressionalliance. org/PDF/depression-disability-and-employment-re port. df) that looked into depression as a disability and how we can improve the way we look at depression and the changes we can make to help reduce job, long term economic inactivity and isolation. Of the 566 people who answered 95% were living with depression and a quarter (148) people had a long term physical health condition. 77% of the people reported that they had not received any support with employment. Gary is living with a type of disability which is difficult, most disabled people are not depressed and do not stay depressed.The prevalence of depression among disabled adults is twice that of healthy older adults (Beekman AT, 2002), the relationship between depression and disability is a two way street with depression there being a link between physical disabilities and depressive symptoms in older adults (Gill TM,Kurland B 2003). There is clear evidence that Gary has on the onset of depression that can be linked to the impact of the loss of his legs in his car accident an d with him now socially isolated and without employment these have now added to the problems over a period of time.In a report by Tania Burchardt called †Enduring economic exclusion' Disabled people,income and work' the study found out that disabled people now make up a large and growing proportion of people who are of working age, it now stands between 12 and 16% (Enduring economic exclusion Page. 4) One telling statistic is the problems that disabled people have getting work, and that one third of those disabled people who find employment are out of work once again the following year.In the report by Tania Burchardt the income statistics show that half of all disabled people have incomes below the general population indicator of poverty after it was factored in about the additional cost that disabled people incur due to their health. There should be no denial of opportunities or restriction of choice based on assumptions that are made due to his disability. Through the social model disability is understood as an unequal relationship within society and there can be no barriers preventing people from gaining equal access to information, education and employment.There are options that perhaps he has not considered and with him feeling socially isolated due to living alone and his lack of contact with the outside world. Conwy Council Adult Social Services promotes day care or opportunities to meet other people and take part in outings and other activities so Gary would be able to haver a much more active life that would could help with the depression he is suffering. If Gary was to obtain the extra help that he is going to need to obtain the goals that he has set himself a consultation with both himself and his family is going to be needed as to ascertain as to realistic goals that can be set. There would be a negative impact o n Gary should he set himself unobtainable goals that he then fails to obtain, this would be damaging to the situation.Statistics show that disabled people have a lower overall level of qualifications when compared to that of able bodied people (Employment of disabled people DoEE) 38% of disabled people have no qualifications compared to only 16% of non-disabled people. The report also concluded that of the 2,015 disabled people that were spoken to (26,000 households in total) 64% strongly agreed and 29% agreed that getting a job was important to them so the statistics show that it is important for disabled people to be in employment. 47% of the economically inactive disabled people who see themselves as being able to work would like a paid job.When speaking about Gary's visions of eventually going back into work his daughter has reservations incase he is taken advantage of by his employer and in the report one in six people (16%) who are economically active stated that they have exp erienced unfair treatment or discrimination in the work place. When looking at the overall statistics you can see that 42% report that that they have been discriminated by a potential employer and 41% by an employer, these figures speak volumes and during any assessment they will need to be presented to Gary so that he can see the real picture in the job market for disabled people.The option of Direct Payments is one that should be explored with Gary and when he is assessed for the amount of care that he needs the options should be discussed at length, direct payments have both good and bad points and do not fit everyone. He will need to consider if he wishes to have the work load of sorting out all the pay, dismissal, redundancy just to name a few of the obligations that he would have to meet as an employer to a personal assistant.The other option open to Gary is to place the care that he requires in the hands of an agency or self employed to take the burden of work away from himself and organise his care around his specific needs. Bringing Gary into the centre of this assessment into his care will empower him to participate (K217 Book 3) Jones and Meleis describe the concept of empowerment as ‘a social process of recognizing, promoting, and enhancing people's abilities to meet their own needs,solve their own problems, and mobilize necessary resources to take control of their own lives (Jones PS, Meleis 199 3).The empowering of Gary will start a process of helping him assert greater control over the factors that affect his health, in the past it was assumed that service users were supposed to be compliant with the directives that were in place from the health care professional's-the biomedical model. In the empowerment model there is a contact between Gary and the provider who take a joint responsibility in terms of agreed upon outcomes, people within the health care profession are obliged to respect Gary and assist him in making decisions.Multiple studies have shown that people that are more involved with the decisions about the care that they receive and also the management of the conditions that they live have better outcomes than those who are not involved in the process (Wagner EH,Glasgow RE,Davis C, 2001) (Greenfield S, Kaplan SH,Ware JE Jr, 1988). Conclusions and Recommendations In this report we have been looking at the situation that Gary is currently experiencing, he is a dis abled person who lost the use of his legs in a car accident some years ago and lives alone now after a divorce.He is looking at the possibility of getting some higher education and hopes to move into employment in the information technology sector, it has already been stated that daughter and son-in-law have reservations about this course of action due doubts about the ex-work colleague of Gary's that is offering him employment and the problems that this will present to her care giving situation.The report has looked at the links between depression, disability and joblessness and we have shown that there are links between these elements that are making Gary's life move difficult but also you must take into consideration that fact that Gary is very socially isolated. The report then looked at the option of direct payments and how Gary's care could be changed so he will have more control and power over his care and become the central focal point so he can move towards a more independe nt life and obtaining the goals that he is setting for himself.The report looked at all the pro's and con's on the direct payment system and all the extra paperwork and responsibility that comes with employing someone and how records must be kept and these funds are public money have to be accounted for. My recommendations are the following, Gary should have his care plan reassessed by Conwy County council Adult social services with the remit of looking at the possibility of changing the level of care that Gary currently is getting and exploring the possibility of Direct Payments as an option.There should also be consultation with his daughter and son-in-law (with the relevant permissions) and have them present to discuss any and all options.
